Please follow the directions below to submit this information to collect from Rockfield Pharmacy. Once submitted, the pharmacist will review your information and a member of the pharmacy team will contact you by phone. As a result of the information provided by you, the pharmacist may arrange for a consultation with you at the pharmacy, offer advice or refer you to another appropriate healthcare professional if the Common Conditions Service is not suitable for you.
